Medical Case Manager: HIV/AIDS Prevention

Job description: HIV Case Manager – Non-profit human service agency in Chester County seeks qualified medical case manager for its comprehensive HIV program. 37.5 hours per week (5 days). Benefits include health insurance, retirement plan, and very generous vacation, sick, and holiday paid time off. Position available immediately for hire. Responsibilities: Provide medical case management to persons with HIV/AIDS of all social, ethnic and economic backgrounds. Assess client needs for services, assist clients with acess to services, make referrals, and complete documentation for client records. Requirements: Bachelor’s degree in human service related field, case management experience, good communication and computer skills. Must have a valid drivers license and reliable transportation. Fluent in Spanish is a plus. Experience providing direct service to people living with HIV/AIDS is preferred. 2+Yrs Case Management experience required. Practicum or work experience in health, welfare, or social service organization, or equivalent in supervised volunteer experience, is helpful. Candidate must have knowledge of HIV/AIDS disease and its treatment, be skilled in relating to minorities and clients of various backgrounds, orientation, and preferences. Must have organizational and data entry skills to meet the documentation and reporting requirements of the funding sources. The position requires professional confidentiality.
